Ah.  THAT I might believe. :-)

In other words, 

 <indexterm>
   <phrase lang="en">Undo dialog</phrase>
 </indexterm>

should be

 <indexterm lang="en">
   <phrase>Undo dialog</phrase>
 </indexterm>

I can't try it out, but it sounds right.  Marco, do you know how to
build an Italian index and see if it works for you?
