It should depend on the dtd in the "<!DOCTYPE" line of the source file.

Try removing <revnumber> and then changing "4.3" to "4.5" (2x!) in 
the "<!DOCTYPE" line. This worked for me.

Then we should consider switching to DocBook-4.5 (Oct 2006).
